Texas Mental Health Services
Texas law requires licensed mental health providers to share the following information with consumers regarding access to records, regulatory oversight, and complaint options.
Accessing Your Health Care Records
You have the right to request copies of your health care records.
To request records, please contact this practice directly and submit a written request. Requests may be submitted by email, through the client portal. or in writing. Records are released in accordance with applicable state and federal privacy laws.

Filing a Consumer Complaint
Consumers may file a complaint regarding mental health services with the Texas Office of the Attorney General, Consumer Protection Division.
